Key Responsibilities:

- Conduct routine home visits, site reviews, and annual assessments to ensure service quality and alignment with individual goals.
- Maintain accurate and timely documentation in accordance with OPWDD guidelines and agency protocols.
- Serve as a liaison between individuals, families, providers, and regional OPWDD representatives to facilitate seamless care coordination.
- Identify service needs and advocate for appropriate supports, including crisis intervention and linkage to community resources.
- Ensure compliance with Medicaid Waiver requirements and support audit readiness through thorough documentation and case management.
Requirements:
- Strong organizational skills with the ability to manage multiple caseloads while maintaining accuracy.
- Excellent interpersonal, verbal, and written communication skills.
- Ability to work independently and collaboratively as part of an interdisciplinary team.
- Knowledge of OPWDD regulations, HCBS Waiver services, and person-centered planning principles.
Qualifications:
- Associate’s degree required; Bachelor’s degree in social work, psychology, human services, or a related field preferred.
- Minimum 1–2 years of experience working with individuals with developmental disabilities, preferably within the OPWDD or Medicaid service system.
- Proficiency in Microsoft Office and familiarity with electronic health records (EHR).
- CPR and First Aid certification preferred.
