About the Role
Aligned Hospice is seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to serve as our After-Hours Triage and Visiting Nurse. This role is essential to ensuring our patients and families receive timely, compassionate support outside of regular business hours. The ideal candidate is a strong communicator who thrives in a fast-paced, autonomous environment and is comfortable managing clinical decision-making remotely as well as in the field.
Key Responsibilities
Answer incoming phone calls after hours and on weekends, providing timely and compassionate response to patients, families, and facility staff. Triage patient needs and symptoms to determine appropriate level of intervention, escalating to on-call clinical staff as needed. Coordinate care with on-call nurses, physicians, and IDG members to ensure continuity of care. Order medications, supplies, and durable medical equipment (DME) as needed to support patient care plans. Fax new medication orders to pharmacies and facilities promptly and accurately. Perform in-person nursing visits throughout San Diego County when visit needs arise outside of scheduled hours. Clearly and thoroughly document all incoming calls, interventions, and visit notes in the patient record. Communicate and collaborate extensively with the Interdisciplinary Group (IDG) to relay patient status and care updates.
Qualifications
Active and unencumbered LVN license in the State of California. Reliable transportation and the ability to travel throughout San Diego County to conduct patient visits. Strong triage, assessment, and clinical judgment skills, particularly in a hospice or home health setting. Excellent verbal and written communication skills, with the ability to document clearly under time pressure. Comfort working independently in a remote, after-hours, on-call capacity. Prior hospice, home health, or triage experience strongly preferred.
